Orders from purchasers not having established a credit history with us require prepayment or specific credit approval; please contact CSI using the e-mail address above or calling (206) 523-4755. All other orders: CSI will ship items requested, with an invoice, within 48 hours of receipt of order--unless we are out of stock.

If pre-paying, please mail a check for the appropriate sum made out to "CSI," along with a copy of this form, to our address below. If paying with a credit card, your highest level of security will be achieved if you fax or call your order in. If nobody is in when you call, you may safely leave credit card information on our voice mail. CSI accepts Amex, VISA, or Mastercard.

Postage and handling charges:
Domestic: For all single book or video orders, add $4.00 for postage and handling (for ordinary book rate). For Priority Mail service (two-three business days for delivery), add $1.00, for any book or video. Discounts: Ten percent discounts on quantities of 10 to19. Twenty percent discounts on orders of 20 or more.
International: For surface rate mailing of single book orders, add $5 for postage and handling to any country. (Surface rate mail can take six-eight weeks.) For air rate mailing of single book orders, add $10 for all countries--except Asia-Pacific ones. Add $12.50 for air postage and handling to the Asia-Pacific region. (Allow one-two weeks for international air delivery.) For all other orders, contact CCSI for a postage and handling quote.
